What treatment did you have for Gleason 9 (G9) no spread?

Had G9 but pet scan and mri found it was local and had not spread. Been on orgovyx for 15 months (will take if for 18 months). Had 28 radiation treatments. I am tolerating everything ok but tired and did gain some weight. My question is what can I expect going forward? I know a G9 is high risk but it had not spread. I am 77 years old and in relatively in good shape. Any advice is appreciated

G9 here too. Age 77. Stopped ADT in January 2025. PSA still undetectable. Checked every three months. I drink daily green tea and white button mushroom capsules, in the hot tea. Had pelvic lymph node involvement too. My treatment lasted 28 months fatigue is daily, so it’s nap time at 1-2 pm. Testosterone is coming back slowly. 59 now. There ya go guy. Hope this helps ya.

Doctor Time works quietly to restore testosterone and relieve symptoms such as fatigue probably related to its lowered levels.

We're in the same boat, per se. G9 here as well. Just got my last 6 month Eligard shot a month ago. PSA currently < .01. Side effects of ADT not pretty for me. Gained about 15 lbs but some folks on here helping me with cardio stuff. I'd say keep as active as possible and eat/drink the healthiest you can within reason. I do partake in wine(the driest of course lol) at times. Let's keep these PSA numbers as low as possible, guys.

Sure helped me. G9 w/mets here too, but only 6 months into treatment. I can see the tunnel but no light yet. Good to hear from folks on the other side.

Green tea for immune boost/ WBMUSHROOMS- keeps PSA down. All my data found on GOOGLE. I walk treadmill 3x/ week to keep heart muscle: exercised too. lol 25 minutes.

I am 78 and while only Gleason 4+3 I have BRCA2 which makes it much more aggressive. Still alive after 16 years.

Were any of these things found in the biopsy intraductal, cribriform, Seminal vesicle invasion, EPE or ECE. (Extraprostatic extensions extra capsular extensions)
. They do make the cancer more aggressive, and increase the possibility of Reoccurrence.

It’s important to keep up with the medication’s if your PSA starts to rise. Definitely get PSA test no less than every three to six months, for life.. I’ve been on ADT for 8 years and ARPI drugs for 6. They keep me undetectable, But I’ve had four reoccurrences. The first one was 3.5 years after a prostatectomy.

You might consider getting a decipher score, With a Gleason nine the reoccurrence rate is high, but it varies a lot. I know people with G9 that have gone 20 or 30 years after treatment before reoccurrence. I know others that have not been so lucky, but staying on treatment the have been able to live a decade and more.
