Feedback wanted: Are you a Mayo patient using an Action Plan?

Mayo Clinic is interested in improving how Action Plans are accessed through the Mayo online patient portal account (Online Patient Services). User design researchers would like to invite 6 Mayo Clinic patients who currently have an Action Plan to discuss your experience with using it, or you can be a caregiver to someone who uses an Action Plan. The interviews will take place in January 2022 during business hours and will last 1 hour.

To take part in the study:
– You must be a Mayo Clinic patient with an account on the patient portal
– You must have one or more of the following Action Plans:
– *Adrenal Action Plan
– *Asthma Action Plan
– *Behavioral Safety Action Plan
– *Bleeding Disorder Action Plan
– *COPD Action Plan
– *Opioid Action Plan
– *Respiratory Care Action Plan

Or you are a caregiver to someone with one of the following Action Plans:
– *Pediatric Adrenal Action Plan
– *Pediatric Anaphylaxis Action Plan
– *Pediatric Diabetes Action Plan

An honorarium will be provided as gratitude for your time.
No private health information will be shared in this study, however, you will be asked to share your experience with using your online Mayo Clinic patient account.

What is an action plan?
An action plan is a custom set of guidelines provided by physicians to patients with the following conditions:
– Adrenal issues
– Asthma
– Behavioral safety
– Bleeding disorders
– COPD
– Opioid usage
– Respiratory care
– Pediatric Adrenal issues
– Pediatric Anaphylaxis
– Pediatric Diabetes

When does my Mayo team give me an action plan?
The care team puts an action plan together with you, the patient, if you have one of the above conditions or situations that benefit from co-managed care and collaboration between your Mayo team and you.

Where do I find my action plan?
On your Patient Portal

What do I do with the action plan? What is expected of me?
The plan outlines what actions you are advised to take for your care. The action plan is tailored just for you and your condition. Actions could be anything from what symptoms your should report to your team or when to call 911. Other actions may include things to do at home to manage your condition.

I looked at my portal and action plan is blank. My care team is listed on there. Unless I'm looking in the wrong place. I had the endobronchial valves placed in 2020. I'm not even sure who to ask a question to when having one? The transplant team? Endobronchial team? Having a plan or knowing who to direct my questions to would be very beneficial. I don't go back until June so I will have to ask about it then I guess.

Susie, you probably are looking in the right place. Your team would've created the Action Plan with you. Yes, please ask your team about this when you return in June.

I currently do not have an action plan but am very interested in having one. I am not due for an appt until June sometime when it will be my yearly visit for the endobronchial valves.

What do you like about having an Action Plan?

I like that my asthma action plan outlines exactly what to do if I have an asthma flare up based on measurable symptoms and when to take different actions.
