Ear Infection

I am so concerned right now. I have had a double ear infection for 6 weeks. This is the second time in 3 years I had it so bad that the doctor is having a hard time pin-pointing a medication that will work. I do not have insurance, so seeing a specialist (ENT) is not an option. I have taken 10 days of Amoxicillan, a 6 day dosepak of Medrol(steroid) I'm working on my 8th day of Augmenten, and now the doctor has prescribed Bactrim ( which I haven't started yet because he wants me to finish the Augmenten. I'm so hesitant to take another antibiotic. The first three didn't do a thing. Now I think I have a very bad sinus infection on top of it. My ears hurt, they feel full and muffled, of course I suffer from tinnitus also and now have vertigo back. My biggest concern is hearing loss. Does anyone know if this is something I should be concerned about. I'm also concerned of other more serious damage from this bacteria sitting in there for so long. Can anyone advise, please...

Is there a state run hospital around that has an ENT DR that could treat it and you not have to pay or only a small amount?
