Don’t know weather to choose evenly or tymlos.
I have si joint pain. For past 3 years have gotten steroid shots every 4 months. April diagnosed with osteoporosis. So no more steroids. My legs buttocks upper back hurt. Cannot take aleve. Am supposed to let my osteoporosis specialist know which medication I want to take. I just don’t know.

Tymlos requires you to self-inject every day for 18-24 months. Evenity is a monthly shot at the doctor's office, for 12 months. The advantage of starting with Tymlos is that, if you still need more bone density when you're done, you can then go on Evenity for a year. So, if you don't mind injecting yourself, starting with Tymlos gives you more options later on, if you need it.

1 ReactionI was diagnosed with high risk osteoporosis in July of 2025 . I was put on Evenity in July 25.
By December of that year I had thyroid levels go up increasing my dosage of Synthroid . My kidney levels rose and have to be measured ev 3 mo . And my blood pressure meds doubled as I began having fluctuations in BP and they were very high . In March of 2026!my internist said I needed scans to check liver and kidney. Reports indicated that I have abnormalities that need to be watched.
In November25 I began having SI joint problems. Ortho did MRI and said diagnosed with sacroillitis. Had to have steroid injections to relieve some of the pain .There were days I could barely walk .
On my annual vision exam in July of 26’ I was told I have a detached retina and macular
Degeneration in intermediate stage . Had a clear visions exam in July of 25.
In July of 26’ dexa showed that
my spine lost density bone etc so Evenity did not work for me .
Through out the year I kept saying to my rheumatologist and my internist,Is this coincidence that since I started
Evenity last July of 25 , all of these issues have developed .
They say it isn’t the Evenity causing the issues . I’m not so sure ! I did report all to FDA and Amgen and have not heard back . From what I’ve read on Social media medical sites there are many others with similar issues.
My dr and his PA vehemently said I need to go on Tymlos . And after reading about this drug I don’t like the possible side effects including development of bone cancer. I refused Tymlos . My internist agreed I should as well . I requested Prolia and was told rheumatologist PA that it will not work for me and waste of time as it won’t build bone like Tymlos . I insisted on the prolia injection and next July I will see if dexa improves from prolia, vitamins, and resistance and weight bearing exercises .
And hopefully another treatment is on the horizon at that point .
Good luck in your decision .
@flop1 There is no risk of bone cancer with Tymlos. The FDA removed that warning a few years ago.
I just made this decision between Tymlos and Evenity, but I think it really depends on your own personal health situation. For me, I had high calcium in my urine, and Tymlos can worsen that. I was scared of Evenity and the black box warning, as I have mild calcification of the arteries. I didn't even consider the mechanism - daily shots vs monthly. If it worked, then hooray. I was leaning Tymlos for the longest time, but chose Evenity and get my first injections this month. Evenity seems to be the one with the best results. But you have to consider all of your health issues. I plugged all of my lab results (minus the personal information), and health info, into AI, and that helped me decide the pros and cons of both
