Combination therapy HRT and Alendronate

I’m a 65 postmenopausal woman .osteoporosis showed up after I lost the estrogen,
I was on HRT for 5 years which halted further bone loss but didn’t build the bone back.

I finished 2 years of Tymlos. I received dexa and TBS results and will test ctx and p1np in blood next week.
Results from Tymlos ;
there was 5.7% increase in spine bone density . ( though T score didn’t move much ) happy Tymlos yielded a decent result for me. Now what to next …,,
Doctor is recommending Fosomax for me.
I asked about resuming HRT again . ( I’ve been off of hormones during the 2 years on Tymlos)
Doctor said it’s a possibility to be on combo therapy HRT and a bisphosphonate but the fosomax dose would need to be lowered.
Anybody have medical knowledge of combo HRT /fosomax ? Best doses of each ? Results ? Risks ?

This is a good question. My Spine is -2.7 and hips -2.0. My Dr suggested Evenity but insurance denied it. So I asked her to do an appeal. Which I know I won’t win so she suggested Prolia which is a last resort for me or Reclast. I had tried Fosomax when I had osteopenia and had severe back pain with 2 nights a week so I stopped it. I recently started HRT in the gel form. I know HRT doesn’t build bone but does stop the bone from breaking down. Which is exactly what Fosomax does. So why would we need both?

Logical good question!
I don’t know why someone would need both .
I don’t know why I can’t JUST do HRT .
The doctor I see for my bones said I need a bisphosphonate .
When I brought up the idea of HRT said it wouldn’t be enough . I think I’ll push back with asking her to explain why

I found this video from Dr Doug Lucas who talks about this exact same thing.

I may have seen a few articles in PubMed.gov on that topic. You could go to that database and see if you find some helpful abstracts.

@2024tymloshelp When I was 60 and needed hip replacements so had not been as activate for a few years, I had mild osteopenia in my forearm. After hip surgery I went on HRT and started the gym..weights etc My bone density remained good for many years according to Dexas it last year even though the Dexa showed low normal, a quantitative CT showed osteopenia average -2.1
So HRT and gym did not continue or do enough.
I continued to go to gym, maintained good weight don’t smoke Blood tests showed no contributing factors So it happens to loose bone density
I’m 82 my husband has severe osteoporosis snd fractures I don’t want thst so I’m doing Evenity.

When I requested HRT therapy, my endocrinologist explained that the side effect of increased thrombus formation can increase the risk of stroke or heart attack. I have heard of an HRT patch that may bypass the liver metabolism resulting in stable/ normal range of clotting factors. I have not asked her this yet but will share what she says.

@paxbar
I specifically asked the NP about risk of stroke or blood clots. She said the patch, gel and cream all bypass the liver. I also googled it and same answer. It gave me a peace of mind.

Are you weight training?

I have to admit that I weight-trained only sporadically during Tymlos treatment due to a torn shoulder
Rotator cuff and another old injury ( not related to osteoporosis).
I walked a lot though .
