cervical stenosis, myelopathy, need some input

I really like some input from any member who knows about cervical stenosis, myelopathy. I am right now quite confused. I have had a severe C5-C6 stenosis (9mm absolute stenosis by birth) with bone spurs for long time. Then came accident 2024 (rear ended, whiplash, concussion) new 2mm retrolisthesis at C5-C6. No major problems after healing from that. I did have gravity flexion x-ray 2025, and learned then I had C3-C4 and C4-C5 anterolisthesis. I assume from accident. 2025 chronic weakness upper back and difficulty holding shoulders straight because arms go numb after an LP . Consulted neurosurgeon at major medical center. He said, I have myelopathy C5-C6, the vertebra above C3-C4 and C4- C5 are unstable. He recommended then to have 3 level ACDF because a next accident could paralyze me. I did want to get second opinion. Then, I had a severe fall 4 month later 2026, this was followed by severe neck and upper back pain, shoulder pains, pain between shoulder blades all ONLY UPRIGHT position- together with stomach sphincter/diaphragm spasms which worsened to abdominal spams --- ALL standing /sitting upright about 30 min. All gone lying down The neck spams turned vice -like, as wells as the stomach spasms. I lie flat 16-18 hours/day. I learned that visceral problems are less common with myelopathy. Bending impossible for me because of severe dizziness, nausea and also stomach spasms. All was explained by neurosurgeon new C3-C4 stenosis after fall . Scared of surgery. Saw a physiatrist I had seen once, who tells me there is “nothing” wrong with my cervical spine while he is looking at recent MRI taken when I am lying flat. He does not take into account what happens under gravity. He has no explanation for any of my symptoms when standing upright. He tells me, he cannot explain the spasms/pain in my neck, shoulders, back despite severe stenosis diagnosed for years now This person has good training, but I feel gas-lit. His opinion is, at time of fall perhaps got coincidentally “stomach hernia” or “stomach acid?”! and an “inner ear disorder”. I am absolutely speechless. His opinion, “surgeons just want to operate”.
