Protect Your Vaccine Record Card

I was just looking at my half-finished vaccination card. Next Sunday is my second vaccination. I can't wait. It's been well over a year that I've been out and about as most of us.

But hold on, what do I do with my card once I'm vaccinated? CNN has some wonderful tips.

First off make sure that all the information on the card is 100% accurate BEFORE you leave where you are getting vaccinated. (name, date of birth, and the date and location of the vaccine -- for accuracy. )

Secondly, take a photo of both sides on your phone, for the first vaccination (if possible) and then the second. Keep a copy on your phone and email a copy to yourself. Then photocopy it and keep at least one copy with your important documents.

Lost cards or very damaged ones need to be replaced. Contact the provider of your vaccinations if you need to replace yours. You might want to jot down the time and place of your vaccination in case you lose your card and need to replace it. Visit your health department immunization information system (IIS). My state has a COVID-19 Vaccine Portal through the State Department of Health.

You can get your COVID vaccine card and print it out from the Mayo Clinic patient portal (Patient Online Services) or the Mayo Clinic app.

1 Log in to Patient Online Services.
2 Select “COVID-19” from the menu on the left side of the home page.
3 Select “COVID-19 Test Results & Vaccine Records” from the COVID-19 page.
4 Select the arrowhead to expand the box with your vaccination records.
5 Select the print icon to the right of the words “COVID-19.”
6 Select the print button in the print window to print your records.

PATIENT ONLINE SERVICES ON THE MAYO CLINIC APP
1 Log in to the Mayo Clinic app.
2 Scroll to the COVID-19 section on the home page.
3 Tap “COVID-19 Results & Vaccinations.”
4 Tap arrowheads to view records.
5 Take a screen shot of the page and follow your usual steps for printing screen shots. To enable screen shots from the Mayo Clinic app on an Android device, tap the three dots on the COVID-19 page to go to “App Preferences” and select “Enable Screen Shots.”

If you have caregiver access to a Patient Online Services account for a patient between ages 13 and 17, you can view and print their COVID-19 vaccination records. Open their account from your Patient Online Services account, and follow the steps above.

if you had the first vaccine three months ago. and have not taken the second one.. any issues / concerns with getting the second shot??

Welcome @rsandler to Mayo Connect. According to the CDC you do not have to start over. Make sure that when you make an appointment that you have your card with you so it can be filled in. There really aren't any studies that show that there are issues about this as yet.

Hi @pvb123 Welcome to Mayo Clinic Connect.

Clinics which administer the vaccinations issue CDC cards at the time of the injections. If you didn’t receive a card, contact your medical provider where you received the vaccine.. They will have your immunization records on file and should be able replace your card.

Did you receive your vaccinations at a Mayo Clinic campus? If so, you should be able to access your records in your Mayo Patient portal and make a copy of the information. Contact the Mayo Clinic where you received the vaccine to get a sanctioned CDC.

I was told Mayo was not providing them, and to refer to the portal for information about my vaccinations. Now I am going on a cruise, and a vaccination card is required for boarding. Photocopies and smart phones are not accepted. How do I get an official vaccination card to use for travel?

Hi @seminolelady I just called Mayo myself to get the straight answer on how to get the official CDC card with the record of your proof of vaccine.

Here is what you need to do:
Call the office of Patient Experience
1-844-544-0036
Request the form for the CDC proof of vaccination and an official card will be sent out within the week.

Or online:
You can get your COVID vaccine card and print it out from the Mayo Clinic patient portal (Patient Online Services) or the Mayo Clinic app.
