Hi @ess77, You can locate and print your vaccination information on your patient portal where you communicate with your health care team and find your medical reports. Here is Mayo's patient portal site:

@amandaburnett Thank you, Amanda. I found the immunization list. That's great to actually have on paper in file in case I need it. Is there any way to get an official Mayo/CDC card with just the Sars vaccine information on it? Like is given out at the shot site? I saw the one my son just got and wonder if a duplicate is available.
