Dry vagina and foamy urine: Internal bleeding?

I have a deviated septum which I've had for years and a lot of drainage coming out of one nostril. I had a cat scan of of my sinuses and all was ok. Dr. put me on two nose sprays Azelastine HCI 0.1% which was 0.3% but I had bloody nose once, and once blood coming from vagina.

So I went to my Urologist and he did a cat scan and said everything looked ok but did a cystoscopy and he saw a little blood and thought it was from my vagina being dry as the bladder was fine and wants me to use some estrogen cream. I never had bleeding there since my hysterectomy in 1986, After having a cat scan with contrast now my urine in bubbly or foamy and they said it was normal and should go away but so far it hasn't. Had my cat scan about 5 days ago. I do not have a GYN here in GA so haven't had a pap smear since I moved to GA in 1997; I didn't think I needed one.

Question is what about this cream they want me to use, and what about a foamy urine. I don't have burning when going to the bathroom. If this is in the wrong area could you tell me where to go to get some sort of an answer. I'm 79 years of age. Also I have AVM's which bleed internally and sometimes need iron infusions. I never see the blood when I bleed internally and had a blood transfusion and iron infusions in 2017. I'm always tired but don't know if it's the meds or what. I'm confused as to what is going on.

Hi @dablues, I can imagine that you are confused about what is going on. Clearly you need to find some answers. Dablues, it sounds to me like the source of the bleeding has not been found. Your doctor suspects vaginal dryness or vaginal atrophy. You can find out more information about vaginal atrophy and treatment in this article:

I have a few questions to consider:
Has internal bleeding from AVM been ruled out? Is your tiredness a new or worsening symptom? Has the foamy urine changed in the 5 days since the CT scan, i.e., gotten less or more?

Hi @dablues sorry to hear of your health issues. I never had any blood in my urine. Are the AVMs the cause of your blood loss? Something is causing your blood supply to be low enough to warrant a transfusion. Anemia can be the cause of your tiredness. My husband had gone to a urologist because hidden blood was found in his urine. The search was on but all the test gave normal results. For him nothing was found and nothing further was done because it was a trace amount of blood loss. Since the urologist gave you a probable diagnosis perhaps seeing a gynecologist will help as a second opinion to rule out anything serious. I would question the doctor that gave you the blood transfusions. How often do you do blood tests and who orders them? Toni

@dablues The nasal spray given to you is classed as an antihistamine. That said, an antihistamine will usually dry up mucus membranes in more than just your eyes/nasal cavity. They can dry out your mouth and vaginal areas also. Your vaginal blood may have been a side effect of the medication, but also a sign of atrophy of vaginal tissues, which is common as we age.

Typically, foamy urine with pain on urination, is a sign of excess protein in the urine. Are you eating a lot of protein? It will shed off in urine. As for the estrogen cream, I would question the wisdom of it, what they hope to accomplish by using that. It may be time to see a gynecologist or your primary for a complete well-woman checkup. As has been mentioned, what was the cause of needing blood transfusions 3 years ago?

I eat the same things I always eat and am not a big eater. What I did add was flaxseed and chia seeds to my diet so I can get off of fiber pills. I don't have a GYN since I moved to GA. Will be calling my urologist back today after coming home from Atlanta. Last checkup on my cataract surgery. I only used the cream once so far. The blood transfusion and iron infusion I got was due to AVM's I have in my body and at that time did not know I was bleeding internally as I never saw blood so my CBC went way down and my primary doctor never did a CBC when he did my blood work and the ER doctor said that is the first thing a doctor should do when having blood work done. So now I see a hematologist every few months. I notice that my white blood cells and red blood cells tend to be low but not sure if it is all the meds I am on. I was told with the AVM's they could be anywhere in my body. I know I have them in my colon, and in my stomach and when the doctor told me about them I asked what to do and he said there is nothing you can do. They might bleed then stop. Evidently mine was bleeding slowly and I didn't know what was going on with me until I was having breathing problems and chest pains, so took myself to the ER and they admitted me.

@dablues I don’t understand. ArterioVenousMalformations have a risk of rupturing and causing more harm. I have not heard of them leaking. You say you have many in your body? Are you sure they are properly diagnosed? If you have many the doctors should be concerned. A CBC(complete blood count) is a normal routine follow up for blood work. Your white blood cell count should remain low otherwise a high amount is indicative of a possible infection. If you are losing blood then you would have a low amount of red blood cells. Remember the red blood cells carry the oxygen so less would cause you to be tired. Your breathing problems may have resulted from the anemia. Hope this helps. Toni

Sorry @dablues, I wanted to ask how much flaxseed and chia seeds you consume daily? Have you discussed this with your doctor? Do you take a blood thinner pill daily? Flaxseeds and chia seeds can act as a blood thinner which will increase bleeding potential in your body. Toni

You may have genital lichens sclerosis. It's still not real common. I was diagnosed 20 years ago when menopause began. I was originally give a testosterone cream to apply to labia and vulvar. I will bleed after intercourse it can go dormant but no cure. It's also more common in menopause/post menopausal women. ust
Just a thought.......

I did not know that. I can't take blood thinners or celebrex for my arthritis due to AVM's. I don't measure how much I eat. I made a pudding with the chia seeds, and I ate 4 oz. I also made flaxseed and chia bread, but haven't had that too much. I never measured. Now I probably can't have that either and will have to go back to fiber pills.
