Elevated Urine Creatinine Clearance

Hello,

Just got back my lab physical and all seems fine except for an elevated 24 Hour urine creatinine clearance test. The range is 0.6 mg / 24 HR to 2.5 mg / 24 HR. My result was 4.04 mg / 24 HR. I usually get my urine and blood updated every 3-6 months in the past 3 years and it has been hovering around 1.8 mg / 24 HR to 1.9 mg / 24 HR. The last test was done 6 months ago and I believe it was 1.9 mg / 24 HR. I am 25 years old, 140 lbs, and 5'10''. All my other serum creatinine, GFR, eGFR, BUN, protein, etc. is fine. With the same urine sample, I had done a 24 HR urine cortisol free test and the creatinine in that test was fine. My random sample urine creatinine clearance was also within range. No prior history of kidney problems. Can this elevated number be just a false positive? Had done kidney ultrasound, renal doppler, kidney MRI, and they came back fine. No swelling in legs or wierd color in urine

I recently had this test because of kidney stones. Virtually all my results were low. My doctor wants me to repeat the test.
Protein can increase your creatinine level, especially red meat. Also, supplements that contain creatinine. Has your doctor mentioned why it is so high? Oh, do you have Diabetes by chance?

The nephrologist does not seem to be concern but i question his abilities to be honest. He was just mumbling during the consultation. I do not have diabetes but can high creatinine clearance be an early marker of diabetes? My fasting glucose and A1C was fine.

Not that I know of. I’d pay closer attention to the
Hemoglobin A1C.
Do you have high blood pressure? That has an effect on kidney health.
I’d run from that doctor and never look back.
I’d ask your regular doctor or urologist. Why were you seeing a Nephrologist?

@nycdave7 With a high creatine level I suggest you make an appointment with a Urologist ask him about your concern as this has to do with the kidney's

@nycdave7 After many years of testing I no longer get excited about an anomaly like your creatinine. Sometimes it happens. There may be many reasons, the most likely (in my opinion) - lab error (mislabeled or entered to wrong patient account). For example, if the lab entered data manually using a numerical patient ID system, you could have been assigned the creatinine from another patient easily by a typo or transposition of numbers. My physicians and I are more concerned with trending and constantly compare my results to see that I remain on trend. That said, with an anomaly such as this my labs would be re-run in a month to assure that it was an anomaly and not a major event and permanent change in my status. Also, if you are already seeing a nephrologist, I see no need to seek out a urologist for your kidney issues. Good luck.

Thanks for the response. It was actually ran twice, second result was elevated but lower than first result. In the past, these results were normal. Had various tests before these urine tests done: abdominal MRI, kidney ultrasound, and renal doppler. Although I have elevated blood pressure, its seems all the imaging tests came back fine. Not sure if horizontal beau's lines on fingernails and chillblains (also toes sometimes turn black temporarily if I press against it, like having my feet on top of another) has any relations to this.
