Do my blood tests results indicate MGUS or MM?

Light chains kappa 26.3 mg/l, lambda 11.0, ratio 2.39; gamma glob 3.7, M spike 3.5, glob total 5.7, mch 33.9, platelets 564, anion gap 9, protein total 9.6 g/dl, globulin total 5.6, and esp igm 4,655 mg/dl.

@barbarian1 I noted that in a post in the Cancer group you credited prayer with this blood analysis being gone.
While I support looking for cure through faith, we partner with science and hope that it will give us some answers and will restore us to health. I suspect that’s why we sign into Connect, to get vetted, data driven information and emotional support from others with a similar diagnosis.
Sadly, the paraprotein in my blood will not spontaneously go away so I rely on my hematologist/oncologist to guide me on this medical journey.
Talk to your doctor. If you have to wait a long time, ask for a telehealth appointment or a phone call. You need answers that we, as lay people who are not physicians, cannot give you.
Will you make that call then let me know how it goes?

Put these results in chatGPT. I have smoldering multiple myeloma & it is a wonderful tool to have.
I would say your MGUS according to your results. I had a bone marrow biopsy BMB & had 10% plasma cells that put me as SMM.

You are under treatment now?
What treatment have you get? Your results was 10% for the beginning or you get in time SMM? My worry is my MGUS not to progress in SMM...

They caught my SMM on a annual checkup. My PCP sent me to a nepherologist due to my EGFR (Kidney function) declining the last 2 yearly checkups
I have had smoldering multiple myeloma for 14 months now. I have bloodwork every 3 months. My 2 PET scans are yearly which showed (both times) no signs of lytic lesions.
You being MGUS only have a 1% chance per year developing into SMM. I have 10 % chance per year developing MM. I wouldn't worry too much you may never go any further.

My BMB was with no sedatives. It depends on each person's pain threshold & how experienced the oncologist doing the procedure whether its tolerable. It was only painful for a couple minutes.

With test results like those, you must have doctors ordering them. I would ask them. I'm sure everyone here means well, but they don't know you. Good luck!

Hello.

Here are the IMWG (International Myeloma Working Group) criteria for Diagnosis of MGUS, Smoldering Multiple Myeloma (SMM) and active MM:

MGUS:
* M protein < 3 g/dl
* Clonal plasma cells in bone marrow (BM) < 10%
* No "Myeloma defining events" (high serum calcium, abnormal kidney function tests, anemia, lytic
bone lesions on imaging studies): (PET scan, MRI, CT )

Smoldering Multiple Myeloma (SMM):
* M protein > or equal to 3.0 g/dl
* Clonal plasma cells in BM > or equal to 10% - 60%
* No Myeloma defining events (see above)

Multiple Myeloma: (MM)
* Underlying plasma cell proliferative disease
AND 1 OR MORE Myeloma Defining Events:
* > or equal to 1 CRAB feature (Calcium, Renal, Anemia, or Bone lesions)
* Clonal plasma cells in BM > or equal to 60%
* Serum free light chain (FLC) ration > or equal to 100
* > 1 MRI focal bone lesions

Since you did not indicate what your BM plasma cells % was, or if you have had any myeloma defining events (high serum calcium, abnormal renal function tests, anemia, or abnormal bone lesions in imaging studies), it is hard to say which category you fall into based on these criteria.

With your M-spike of 3.5 g/dl and IGM of 4655 mg/dl, and Free light chains ratio of 2.39, so far, (just based on these results only,) you *might* fit into either the MGUS or SMM categories. But that cannot be determined with certainty until you have more results, such as Bone Marrow Biopsy (BMB) to determine plasma cell %, serum calcium, renal function testing (BUN/Creatinine/GFR), CBC (to assess anemia) and/or imaging studies (CT/PET/MRI) to look for lytic bone lesions.
