I filled out the initial application for care, received a response that said they were looking it over. Then after a week or so I received more forms to complete (very long forms with questions about my symptoms and what I’ve done about them. Also about when and where I have had any medical care. After quite a number of weeks I received an acceptance form and a date to do an online visit with the initial intake nurse. She gave me a choice of dates to come to Mayo.
