NASH and cirrhosis of the liver.

Posted by mollyb1968 @mollyb1968, Sep 1, 2016

I have type 2 diabetes and have been diagnosed with chronic non-alcoholic steatohepatitis with mild intracytoplasmic cholestasis. Minimal lobular actvity. Scattered lymphocytic infiltrates noted within the portal tracts with minimal hepatocyte necorsis and mild portal fibrosis. Due to also having portal hypertension, gastric varices and stomach polyps, it has been suggested that I may have cirrhosis of the liver as well. Does anyone else have this diagnosis and should I be greatly concerned? Is there treatment for this? Any help would be appreciated.

Hi Rosemary, I haven't talked with you for a while and I am not sure how to get to your page. I posted a while back that my doctor said I was stage 4 fibrosis, which is cirrhosis. However, she told me last month that I was stage 4 cirrhosis but my liver was compensated. I am scheduled for another EGD on March 16th. My tests are all running the same so what makes it change to stage 4 cirrhosis instead of, maybe, stage 1 cirrhosis. She said there were a number of factors such as portal hypertension and gastric varices. Does this make sense and am I understanding correctly. Thank you in advance for your help.

Hi @mollyb1968, thanks for this post! I was thinking about you last week, and I never got around to posting it to you.
It looks like you are getting some good continuing care by your doctor. All this stuff, makes sense, to me only because it is all so interconnected with the liver. I assume that you are currently taking meds to keep portal hypertension in check. The EGD will give the medical team a clear picture if these meds are doing their job well enough. The varices are related to the hypertension which is related to liver and cirrhosis. I've had EGD to look for esophageal varices, and later had one to repair the bleeding varices. You can take comfort knowing that your doctor is monitoring this in order to prevent a potentially serious complication.
I continue to encourage you to take your list of questions to your doctor. And to ask for an explanation in everyday - nonmedical language. I have also found it helps me to repeat it back to my doctor to be sure I understand correctly.
Molly, how are you feeling these days?
Rosemary
PS I'll be thinking of you on Mar. 16. Let me know how it goes.

Thank you so much for your encouragement. It helps me feel more sane. I stay very tired and weak. Sugars have been dropping quite a bit lately and we are working on that. Otherwise, not too bad. I hope you are well and I will keep you posted on the EGD. Again, thank you for your encouragement. MollyB1968

NASH is caused by fat accumulating in the liver, damaging tissue. There are multiple stages of NASH. If it progresses, the end/worst stage is cirrhosis, which is scarring of the liver. If it continues to progress, it leads to liver failure. The good news is that the liver has the potential to repair itself prior to cirrhosis. I'm a relatively new liver transplant recipient. I did not have NASH, as my cirrhosis was caused by alcohol. But in my case as well as yours, the physiological culprit is fat in the liver. I've learned quite a bit about liver health over the past few years, as I'm extremely interested in keeping my new liver in good shape. If you're interested in what you can do slow, stop or reverse the progression of your disease, I'd be more than happy to share my opinions.
